FPPC Forms, Manuals and Instructions
The following forms are provided by the government in the Adobe Acrobat Portable Document Format (PDF). You will need the Acrobat Reader installed on your computer to view these forms.
Neither your local filing officer nor the FPPC accept these forms as electronic filings. Instead, the Secretary of State requires that electronic filings be transmitted through specific data formats that the government has developed. For more information about the Secretary of State and electronic filing see the Secretary of State’s Political Reform Division.
Important: For committees required to electronically file, the Secretary of State requires a committee officer to complete the Application to Obtain Logon Password and Authorization of Vendor form and copy it onto official committee letterhead. The Secretary of State accepts the form by fax. For your convenience we have linked the Filing Authorization Form below.
